By accepting these terms and conditions and submitting the request form online, the obligor (sole or joint) authorizes North Side Federal Savings and Loan Association (the association) to initiate a direct debit against the account listed on the submitted request using the information contained on the submission. The individual submitting said request certifies to the association that they have the ability to authorize this direct debit against the checking or savings account, acting alone. It is understood that this request will be acted on when received and will continue indefinitely until revoked in writing and received at the offices of the association.

Payment will not be considered to have been made until funds have been received. . Please allow up to seven business days for the initial transaction to process.

The account owner agrees to hold the association harmless for any actions relating to the request for transfer.
