By accepting these terms and conditions and submitting the request form online, the tranferor (sole or joint owner of the account) authorizes North Side Federal Savings and Loan Association (the association) to initiate a transfer withdrawing money from the transferor's account at the assocition and originate an ACH direct deposit request to the tranferor's account at another financial institution. The individual submitting said request certifies to the association that they have the ability to authorize this direct debit against the donor checking or savings account, acting alone and that the information contained in the request is accurate.

The account owner agrees to hold the association harmless for any actions relating to the request for transfer.
